Click to DM ads are Instagram and Facebook ads that open a direct message (a DM) with your clinic when someone taps them. No website, no form. The conversation is the destination. A med spa should use them when it can answer those messages fast and turn the chat into a booked consult.
The format is simple. The work is in what happens after the tap.
How click to DM ads work, step by step
A click to DM ad moves a person from their feed into a private chat in one tap. Meta describes ads that click to message as ads that "send people who click on your ad directly into a conversation with your business." The chat can open in Messenger, Instagram or WhatsApp (Meta Business, 2026). The format is growing fast. Meta says its US revenue from click-to-message ads grew more than 50% year over year by Q4 2025 (Meta, 2026). Ryder Scale is not affiliated with Meta.

- A person sees your ad in their Instagram or Facebook feed.
- The button says "Send message" or "Chat now" rather than "Learn more".
- The tap opens a DM thread with your clinic's account. A greeting you wrote in advance appears right away.
- The person asks a question or picks a reply. Your side answers.
- The chat moves toward a booking: qualifying questions (simple checks that the person fits the offer), available times, a card to hold the slot, and a confirmation.
- The consult is written into your booking system.
The ad setup controls steps 1 to 3. Your inbox controls steps 4 to 6. That is why the same ad works for one clinic and fails for another.
Click-to-DM vs lead form vs landing page
The main difference between the three destinations is where the conversation happens and who starts it. Our guide to Instagram ad destinations for med spas goes deeper on each route.
| Compare | Click-to-DM | Lead form | Landing page |
|---|---|---|---|
| What the tap opens | A DM thread with your account | A short form inside the app | A page on your website |
| What you receive | A live conversation | Name, phone and email | A visit, and a booking only if they finish it |
| How much speed matters | High: a slow reply ends the chat | Medium: call within minutes | Low: the page does the work |
| Best fit | One clear offer and fast replies | Clinics that are strong on the phone | Clinics whose booking page already converts |
Click to DM ads fit a clinic that wants warmer inquiries and has the coverage to answer them.
What your clinic needs ready before running click to DM ads
Before running click to DM ads, your clinic needs five things in place.

- One offer. Pick a single front-end offer, such as a consult for a named treatment at a stated price. (A front-end offer is the first thing a new patient buys, not your full menu.)
- Written answers. List the 15 to 20 questions people ask about that offer: price, what is included, how long it takes, who performs it. Write the approved, non-clinical answer to each. Our med spa DM scripts give ready answers to the 20 most common ones.
- Coverage. Decide who answers, and during which hours. Ads run at 9pm on a Sunday. If nobody is answering then, pause the ads at those hours or use a tool that can reply.
- A booking path. The chat must be able to end in a chosen date and time in your real calendar. "Call us tomorrow" sends the person back to the start.
- A handoff. Agree what happens when the chat leaves the script: a clinical question, a complaint, an existing patient. Decide who gets it, and how fast.
Who should answer the DMs
The right person or tool for your DMs depends on two things: how fast you can reply, and what each reply costs you.
Front desk. Your team knows the clinic best. The trade-off is that they are also checking people in and answering the phone, so DMs wait, and night DMs wait until morning.
Virtual assistant (VA). A VA is a remote worker who covers your inbox for set hours. The trade-off is training, turnover, and the gap between what the VA says and what your clinic would say.
AI receptionist. An AI replies in seconds at any hour. The trade-off is scope. It needs a tight set of approved answers and a clear rule for when to stop and hand off. Our guide to AI receptionists for med spas covers what to ask before you turn one on.
At Ryder Scale, we run the AI only on DMs that start from the ads we manage. It answers questions about that one offer and asks non-clinical qualifying questions. It offers available times and takes a card on file to hold the slot. It then writes the confirmed consult into your booking system (Boulevard, Zenoti, Vagaro, Mindbody, Aesthetic Record or Square). Anything outside that flow goes to your team. The rest of your inbox is untouched.
When click to DM ads are the wrong choice
Click to DM ads are the wrong choice when your clinic cannot answer within minutes, or when your offer needs a long explanation before anyone would message. A DM is a promise of a quick reply. Break that promise and the person moves on. One older study measured phone calls to web leads, not DMs. The odds of reaching a lead were 100 times higher when the call came within 5 minutes instead of 30 (InsideSales.com and MIT, 2007).
Two more risks to weigh:
- Unqualified messages. A tap is easy. Some people message with no plan to book. Your qualifying questions have to screen them politely.
- No contact details by default. A DM gives you a username, not a phone number or email. Ask for them in the chat.
Compliance notes for med spa DM ads
Keep every answer in the DM non-clinical. The chat can state what the offer includes, what it costs, how long the visit takes and when the clinic is open. It cannot say whether a treatment is right for that person, suggest a dose, or comment on a photo they send. Those questions go to a licensed professional at your clinic.
Some states require a business to tell people when they are talking to an AI. In states with AI disclosure laws (California, Illinois, Maine, Utah and Colorado) our AI identifies itself as an AI assistant in its first reply. Check your own state's rules with your attorney.
Have your agency check the ad creative (the image, video and words) against Meta's advertising rules before launch.
What to measure on a click-to-DM campaign
Measure each step from tap to paying patient, and keep the definitions fixed.
- Conversations started: unique people who opened a DM from the ad.
- Conversations answered within five minutes: your coverage score.
- Qualified booked consults: a date and time chosen, with a card on file. That is the definition we bill on. It is not a promise that the person attends or buys.
- Attended consults and new paying patients: supplied by your clinic from the booking system.
Our guide to tracking med spa marketing ROI shows how to turn those counts into cost per booked consult and cost per new patient. (ROI means return on investment.) We have no published case studies, so we do not quote conversion rates.
Frequently asked questions
Are click to DM ads the same as click to message ads?
Yes. Meta's name for the format is "ads that click to message." Agencies and clinic owners say click-to-DM when the conversation opens in Instagram Direct, because Instagram calls its messages DMs. The same ad format can also open Messenger or WhatsApp, depending on where your clinic messages with patients.
Do Instagram DM ads for med spas need a landing page at all?
No. The DM is the destination, so the ad runs without a website page. You still need a booking system the chat can write into, and a clear offer the chat can explain. Many clinics keep a landing page for other campaigns and run DM ads alongside it.
How fast does a clinic need to reply to an ad DM?
Reply within minutes, not hours. The person tapped your ad while scrolling and is still holding their phone. If the first reply comes the next morning, the chat has gone cold and you have paid for the tap anyway. Set your coverage hours before launch and staff them.
What does a click-to-DM marketing agency actually do?
A click-to-DM marketing agency builds the offer with you, produces the ad creative, runs the campaign in Meta's tools, and manages the conversation that follows. Some agencies stop at the ad and leave the inbox to the clinic. Ask which parts are included and who owns the handoff before you sign.
If you are still comparing routes to new patients, our overview of med spa marketing strategies puts click-to-DM ads next to the other options. Our med spa marketing agency service runs Instagram and Facebook ads for one front-end offer per clinic, with an AI receptionist on the DMs those ads start. You pay Meta directly for ad spend, and you pay us with no retainer and no setup fee, only per qualified booked consult. Book a free 20-minute audit to see whether that scope fits your clinic.
